Navy Health Ltd ABN 61 092 229 000 (Navy Health, we, our, us) is collecting your personal information on this online membership form for the purpose of processing your membership application with Navy Health, and if your application is approved, to provide our products and services to you (such as your insurance coverage, assessing your claims and paying your benefits under a health insurance policy). As an insurance provider, the personal information we collect also includes ‘health information’ which is considered ‘sensitive information’ under Australian privacy laws.
We use personal information that you provide to us for that purpose and to generally manage our relationship with everyone insured under the relevant policy. In order to provide the best possible service to our members, we may also use the personal information collected to consider any other applications which may be made to us by those insured, designing new insurance products and services, for research and analysis and to perform administrative functions as well as to comply with any legal obligations we might have.
We may also use the personal information provided to contact you or other individuals insured under the relevant policy, including to let you know about products and services we believe may be of interest, such as health management programs.
From time-to-time we may use the personal information provided to contact you and other individuals insured via, email, phone, SMS, mail and other channels to send out carefully selected materials about our products, services and special offers and promotions as well as those provided by our trusted partners, such as Defence Bank.
If the personal information on this form is not collected, we may not be able to process your membership application with Navy Health, and you may not be able to access some or all of the products or services that you request of us.
If you are taking out an insurance policy for more than one person, such as under a family or couples policy, please ensure that you have the consent and authority to provide us with the personal information of that person or those people.
In order to provide our products and services to you, we may disclose (share) your personal information to third parties or organisations, such as health service providers, other health insurers and third party service providers such as IT service providers.

Our Privacy Policy also includes information about how you can opt-out of marketing communications from us, how you may access your personal information or seek correction of your Personal Information. It also includes information about how you may make a complaint about how your personal information has been handled.
